NAD infusions get talked about as if they were a wellness add-on. They are not. NAD is a coenzyme that participates in cellular energy and DNA repair, and high-dose infusions affect how the body produces energy. That is a real intervention. We treat it as one.
What we run before your first drip
- →Comprehensive metabolic panel
- →Complete blood count with differential
- →Liver and kidney function
- →Thyroid panel (TSH, free T3, free T4)
- →Vitamin B12 and folate
- →Vitamin D
- →Iron studies
- →Hemoglobin A1C if there is any reason to suspect metabolic dysfunction
We add to this depending on the history. If a patient is over 50, or has cardiovascular risk factors, we may add lipids and inflammatory markers. If they are taking medications that interact with renal clearance, we adjust the workup.
What we ask
We take a full medication and supplement list. We ask about pregnancy or attempts to conceive. We ask about a history of cancer, even if treated and in remission, because the data on NAD in the context of active or recent malignancy is not where we want it for safety. We ask about cardiac history and any prior reactions to infusions or contrast.
Who we do not drip
Pregnancy or breastfeeding. Active malignancy. Significant renal or hepatic impairment. Anyone with a history of severe infusion reaction. Sometimes the answer after intake is 'not today, here is what we would want to clean up first', and we move on to other options. That is a good outcome.
On the day
Plan for two to three hours. NAD is infused slowly because the side effects (chest tightness, nausea, anxiety) are dose-rate dependent. Slow drip, frequent check-ins, and a quiet room make the experience tolerable. Most patients work, read, or nap.
After
We talk about what you noticed in the first 48 hours and adjust the cadence from there. Some patients respond best to a loading series over a few weeks, then maintenance. Others do single drips quarterly. There is no universal protocol. The protocol is yours.
